A little under-researched I think, but then again you can't expect much from these half arsed TV shows. In regards to the impalement, most of the time (and it is still in practice today unfortunately), the stake would protrude from the collar bone region rather than straight through the mouth, ergo it was rather easy to do and keep them alive. Also quite often the stake was inserted only partially and when the victim was raised they would slowly impale themselves whilst writhing in agony. :D
