To all of you saying it's the spotter's fault for not warning the driver, he probably did warn him as you can see the brake lights come on just before the collision but because the driver was going too fast he didn't have time to stop. So the driver probably shares some fault too.

my observation is that driver made a good turn ,it was just a load force on tail..and of course speed ,but driver was confident about speed while turning ..

thats tremendous tremendous amount of wieght for a semi dolley way too much for comfort youd have to turn L or R very slow and stop and make sure the back wheels are still faced straight and that they did not pivot off track to the L or R i have had to readjust and adjust hundreds of times doing this but no longer do this

that was unavoidable the dolley moved to the right on the left turn because they were not tight and he wasn't paying attention after every turn you have to maje sure your back wheels are still pointed straight especually after turning the wieght causes them to move to the left or right depending on the turn biggest fault was it was unavoidable the immense weight of the beam would require you to readjust the dolley or back tires after every turn or find something strong other than a semi dolley that pivots with the weight

totally the truck drivers fault. way to fast and veered right. you can see him try to pull it back left at the last second. not only that but driving that fast doesn't give your pilot cars time to fart let alone react. leave heavy haul to trained professionals.

The dolly veered off or not , the driver took the turn too fast n not enough space to clear the bridge, and not enough time for the flag man to warn him ( or maybe he did ) cause he breaked the last seconds. rookie mistake for an expert driver..happens to everyone.

The dolly moved out of position. Spotter should have been on the radio warning the driver and the driver should have been watching his load.

looks as though everyone was doing their job..no audio of course. seems as though the rear steering axle didn't straighten out properly after the turn. perhaps a malfunction

the load twisted on the turn and moved from center to the left of center making the pup turn right when going straight. the spot truck should have told the driver befor finishing the turn.
